If you are enrolling for the fall semester, it is very helpful for us to know by the beginning of August. However, we recognize that needs and decisions can change. We welcome you and your child at any point during the year.
As you can imagine, there is some paperwork involved in the enrollment process. You have three options:
Stop by the office to pick up a packet of forms, or call us and we will mail paper copies to you. (See site footer for contact information.)

#1 Registration Form
Please complete the online registration form here.
#2 Birth Certificate
Please email or send a copy of the birth certificate for each child you are enrolling for the first time at St. John’s Lutheran School.
#3 Health Appraisal
Have your pediatrician complete a State of Michigan Health Appraisal for each child. You can download the form here.
#6 Deposit
Please drop off or send your non-refundable $250 deposit. This deposit will be applied to your tuition bill.
#4 Immunizations
Please complete the Immunization Disclosure form here.
#5 Scholarship
Please complete the Scholarship Request form if you are requesting financial assistance.
